This page contains league Administrative paperwork in Microsoft Word DOC format and Adobe Acrobat PDF format. If you have Microsoft Word on your computer, the Word documents can be filled in online, printed, and mailed. They may also be filled in online and saved to your hard drive for later e-mailing to the appropriate league official. Instructions for downloading the Adobe Acrobat PDF files are below the table containing links to the PDF files.

These forms and documents require Adobe Acrobat Reader to open them. Adobe Acrobat Reader is FREE software from Adobe. If you don't already have Acrobat Reader, get your copy by clicking on the button.
After downloading the reader to your hard drive, it is necessary to install the program by finding the file you downloaded and double-clicking on that file to start the installation.
After Acrobat Reader is installed, you can print copies of the forms you need by double clicking on the text link above. When Acrobat Reader opens the form, be sure to use the "PRINT" command just above the document. DO NOT try to print the document using the print command on your browser, as this will only give you a blank sheet of paper.
